6010.25 Ministry Event Request Form

This form is designed to streamline the submission and approval process for all ministry-sponsored events. It ensures that complete and accurate information is provided regarding event details, scheduling preferences, facility needs, marketing requests, and required approvals.

To allow adequate time for planning, coordination, and promotion, all ministries must submit event requests at least 60 days prior to the proposed event date.

If promotional materials have already been created, please upload the event flyer with your submission. If a flyer has not yet been designed, include a brief sketch or written concept outlining your vision for the flyer (theme, colors, key details, and preferred messaging).

Submitting this form does not constitute approval. Final confirmation will be communicated once the request has been reviewed and processed.
