For SUNDAY Pickup submit no later than Wednesday by noon

Submission of this form does not guarantee pickup. All requests are subject to van availability, driver availability, routing capacity, and final confirmation by the Transportation / VAN Ministry team.

Once your request is reviewed, a ministry representative will contact you by phone and/or email to confirm your pickup time and location. If you do not receive confirmation, please do not assume pickup has been scheduled.
